TEAM LEADER'S DECLARATION

 

I, the undersigned Team Manager, certify that our Competitors named on the Team Name List Form are physically fit and are water competent or able to swim at least 50 metres wearing light clothing. I also confirm that there is no known  medical reason why those named should not take part in the Race Programme conducted by the Penang International Dragon Boat Festival Organising Committee (hereinafter named "Organiser").  
 
I hereby discharge the Organiser as well as any other individual or organisations connected directly or indirectly with the Championship  from any responsibility in the event of injury, death or loss of property incurred during, as a consequence of, or whilst travelling to or from the event.  

I hereby expressly agree and will be responsible to my team members' conduct and also recognise and accept: 

a. The mandatory character of the Rules and Regulations of the Organiser in their current version and as amended from time to time, and I agree to uphold and abide by them without reservation.

b. The obligation to respect fundamental ethical principles and to act within the implicit nature and intent of the Organiser, that is, to always compete/participate honourably and uphold the principles of good sportsmanship, fair play and non-discrimination and to behave, in all circumstances, in an appropriate manner.

c. To properly wear, at all times, while participating the event the buoyancy vests provided.

d. To grant the rights to the Organiser and its affiliates to use our names and pictures to advance the interest of the Championship and also grant the rights to preserve the entry and results data obtained and store in a computerized data system, and to use such data to further the interests of the Championship.

e. In the event of injury, accidents, and/or illness during the competition; will accept the medical treatment that may be deemed necessary by the Organiser.

f. That any personal behaviour that could result in action being taken under the Organiser Disciplinary Code, is deemed to be NOT appropriate.  

I undertake full responsibility for the eligibility of my team members.
